How do we enroll our unit?....easy!  Just follow these four steps.

Step 1 - Download, print and complete these three forms

  1. Participation Agreement
  2. W-9 Form
  3. Electronic Deposit Authorization Form

Step 2 - Submit the complete forms by:

  • Scaning and emailing them to ctptsa.discountpower@gmail.com
  • or fax them to (203) 909-6327, attention: Doug Newman, CT PTSA

Step 3 - Obtain your unit's Coupon Code
After submitting the three complete forms (step 1 and step 2 above)  your unit's Coupon Code will be emailed to you from CTPTSA.DiscountPower@gmail.com within 24 hours if you submit the forms by email and 48 hours if submitted by fax. 

Step 4 - Tell the world!
Once you have your Discount Code, encourage your members to enroll and to tell their friends, family, local businesses, etc. to enroll too at www.CTPTSADiscountPower.com and make sure they use your unit's Coupon Code!
