What is an 'Officer Information Sheet', and why do I need to Fill One Out?
By completing an Officer Information Sheet (OIS) form on an annual basis, you are assisting in the communication process between the state office and your local members! Communication is the key to running a great organization and is the backbone of successful events and programs.An Officer Information Sheet must be filed with the state office (CT PTA) on an annual basis, preferably immediatlely after your unit or council's elections of officers. This information is critical to so that the state offic can send correspondence to the correct officer(s).
The OIS information is entered in the CT PTA database and is used only by the CT PTA, it is not provide to any other third party.
Unless otherwise noted on the form, both the CT PTA and National PTA correspondence will be sent to the "PRESIDENT". All other officers listed on the form will receive the CT PTA Bulletin and other important mailings pertaining to their positions and chaired programs, events, and award opportunities from CT PTA.
It is the responsibility of the President (or his/her designate) to complete and submit this form by
July 1 of each year. An OIS form will be mailed to all unit and council Presidents in April or May of each year.
You can also fill out an Online OIS Form, or print out the paper version, both of which are part of the Members Only area of the website.
